Insulin lispro (recombinant DNA origin produced in E. coli)
Indications/Uses
DM in adults & childn who require insulin for the maintenance of normal glucose homeostasis. Initial stabilisation of DM.
Dosage/Direction for Use
Individualised dosage by SC inj in the upper arms, thighs, buttocks, or abdomen. Vial/cartridge Can be administered by continuous SC infusion pump, or by IV.
Administration
Should be taken with food: Administer w/in 15 min before or immediately after meals.
Contraindications
Special Precautions
Transferring to another type or brand of insulin; change in strength, brand (manufacturer), type, species, &/or method of manufacture. Conditions which may make early warning symptoms of hypoglycaemia less pronounced or different eg, long duration of diabetes, intensified insulin therapy, diabetic nerve disease or medications eg, β-blockers. Risk of hyperglycaemia & diabetic ketoacidosis w/ inadequate dosages or treatment discontinuation, especially in insulin-dependent diabetics. Increased insulin requirements during illness or emotional disturbances. Reduced insulin requirement in patients w/ renal or hepatic impairment. Increased insulin resistance in patients w/ chronic hepatic impairment. Dose adjustments may be necessary in case of increased physical activity or change in diet. Reports of cardiac failure in combination w/ pioglitazone. Perform continuous rotation of inj site to reduce risk of developing lipodystrophy & cutaneous amyloidosis. Patients who are blind or have poor vision must get help/assistance from another person who has good vision & is trained in using the insulin device. May impair ability to drive & use machines. Pregnancy & lactation.
Adverse Reactions
Local allergy; redness, swelling & itching at inj site. Hypoglycaemia.
Drug Interactions
Increased insulin requirements w/ OCs, corticosteroids, thyroid replacement therapy, danazol, β2-stimulants (eg, ritodrine, salbutamol, terbutaline). Reduced insulin requirements w/ oral hypoglycaemics, salicylates (eg, ASA), sulpha antibiotics, certain antidepressants (MAOIs, SSRIs), certain ACE inhibitors (captopril, enalapril), AIIAs, β-blockers, octreotide, alcohol.
