Dosage/Direction for Use
CRAVIT i.v. should only be administered by intravenous infusion.
It is not for intramuscular, intrathecal, intraperitoneal, or subcutaneous administration.
CAUTION: RAPID OR BOLUS INTRAVENOUS INFUSION MUST BE AVOIDED. Levofloxacin Injection should be infused intravenously slowly over a period of not less than 60 or 90 minutes, depending on the dosage. (See Precautions.)
The usual dose of CRAVIT Tab. or i.v. is 250 mg or 500 mg administered orally or by slow infusion over 60 minutes every 24 hours or 750 mg administered orally or by slow infusion over 90 minutes every 24 hours, as indicated by infection and described in the following dosing chart. These recommendations apply to patients with normal renal function (i.e., creatinine clearance >80 mL/min). For patients with altered renal function, see Patients with Impaired Renal Function: Table 18 as follows. Oral doses should be administered at least two hours before or two hours after antacids containing magnesium, aluminum, as well as sucralfate, metal cations such as iron, and multivitamin preparations with zinc or didanosine, chewable/buffered tablets or the pediatric powder for oral solution.
