Methylphenidate HCl
Indications/Uses
ADHD in childn ≥6 yr, adolescents & adults up to 65 yr.
Dosage/Direction for Use
Methylphenidate-naive patients or patients on stimulants other than methylphenidate Adult 18-65 yr 18 mg or 36 mg. Childn 6-12 yr & adolescent 13-17 yr 18 mg. Max: Adult & adolescent >13 yr 72 mg daily. Childn 6-12 yr 54 mg daily. Patient currently using methylphenidate 5, 10, 15 & 20 mg bd or tds is converted to 18, 36, 54 & 72 mg every morning, respectively. Dosage may be increased in 18 mg increments at wkly intervals. To be taken once daily in the morning.
Administration
May be taken with or without food: Swallow whole, do not divide/chew/crush.
Contraindications
Hypersensitivity. Concomitant or w/in 14 days of MAOI therapy.
Special Precautions
High potential for abuse & misuse; assess patient's risk for abuse, misuse & addiction before prescribing & throughout treatment. Avoid use in patients w/ known structural cardiac abnormalities, cardiomyopathy, serious cardiac arrhythmia, CAD, or other serious cardiac disease. Monitor patients for HTN & tachycardia. May exacerbate symptoms of behavior disturbance & thought disorder in psychotic patients. Screen patients at risk for developing manic episode (eg, comorbid or history of depressive symptoms or family history of suicide, bipolar disorder or depression). Discontinue if psychotic or manic symptoms occur in patients w/o prior history of psychotic illness or mania; presence of seizures. Patients who develop priapism; peripheral vasculopathy including Raynaud's phenomenon. Closely monitor growth suppression in ped patients; interrupt treatment if patients are not growing or gaining height or wt. Do not administer w/ pre-existing severe GI narrowing (pathologic or iatrogenic). Periodic hematologic monitoring is advised during prolonged therapy. Patients w/ history of abnormally increased IOP or open angle glaucoma. Regularly monitor for onset or exacerbation of motor & verbal tics, emergence or worsening of tics or Tourette's syndrome during treatment. Pregnancy & lactation. Elderly >65 yr. Not to be used in childn <6 yr.
Adverse Reactions
Adult Decreased appetite, headache, dry mouth, nausea, insomnia, anxiety, dizziness, decreased wt, irritability, hyperhidrosis. Ped Upper abdominal pain.
Drug Interactions
Not to be used w/ MAOIs (currently or w/in the preceding 2 wk). Possible increase in BP w/ vasopressor agents. May inhibit the metabolism of coumarin anticoagulants, anticonvulsants eg, phenobarb, phenytoin, primidone, certain antidepressants eg, TCAs & SSRIs. May increase the risk of sudden BP & heart rate increase during surgery w/ halogenated anesthetics. May increase the risk of extrapyramidal symptoms w/ risperidone.
