Per 500 mL (after mixing the 2 soln from upper & lower chambers) Electrolytes: Na 17.5 mEq, K 10 mEq, Mg 2.5 mEq, Ca 2.5 mEq, Cl 17.5 mEq, sulfate 2.5 mEq, acetate 8 mEq, L-lactate 10 mEq, citrate 3 mEq, P 5 mmol, Zn 2.5 micromole. Carbohydrate: Glucose 37.5 g, glucose conc 7.5%. Amino acids: Total free amino acids 15 g, total nitrogen 2.35 g, essential/non-essential amino acids: 1.44, branched-chain amino acids 30% (w/w). Vit: Thiamine Cl HCl 0.96 mg (thiamine equiv of 0.75 mg). Total cal: 210 kCal, nonprotein cal: 150 kCal, nonprotein cal/nitrogen: 64. Indications/Uses
Provision of amino acids, electrolytes, vit B1 & water to patients w/ mild hypoproteinemia or mild malnutrition due to inadequate oral intake, & before & after surgery.
Dosage/Direction for Use
Adult Administer 500 mL over 120 min, infused via a peripheral vein. Max dose: 2,500 mL/day. Adjust dose according to patient's condition, body wt & age.
Contraindications
Hypersensitivity to thiamine Cl HCl. Patients w/ hepatic coma or at risk of developing hepatic coma; CHF; severe acidosis (hyperlactacidemia etc.); hyperkalemia or Addison's disease; hyperphosphatemia or hypoparathyroidism; hypermagnesemia or hypothyroidism; hypercalcemia; abnormal amino acid metabolism; reduced urine output due to obstructive uropathy; serious renal disorder, azotemia, or oliguria (for all, patients on dialysis or hemofiltration are excluded).
Special Precautions
Should be used only for short-term nutritional therapy. Limit use to 3-5 days & institute oral/enteral nutrition or other regimens as soon as feasible to do so when used alone in post-op patients. Maintain urine output at >500 mL/day or >20 mL/hr before administration. Use an alternate infusion site or discontinue administration if vascular pain occurs. Carefully observe inj site for extravasation. Avoid photochemical decomposition of vit B1 in the soln. Administer carefully in patients w/ known hypersensitivity to drugs; CV dysfunction; acidosis; DM; renal dysfunction & those on dialysis or hemofiltration w/ serious renal disorder, azotemia, or oliguria; hepatic dysfunction. Should be used in women who are or may be pregnant only if expected therapeutic benefits outweigh potential risks. Avoid use in nursing mothers. Safety in childn has not been established. Reduce infusion rate in the elderly & in critically ill patients.
Adverse Reactions
Vascular pain, phlebitis. Nausea, vomiting; chest discomfort; elevated AST, ALT, alkaline phosphatase or total bilirubin levels.
