Indications/Uses
Flu in adults & childn including full-term neonates. Prophylaxis of flu in adults & childn ≥1 yr.
Dosage/Direction for Use
Flu Initiate w/in 1st or 2nd day of onset of symptoms. Adult & adolescent ≥13 yr 75 mg bid for 5 days. May give susp to patients unable to swallow cap. Childn weighing >40 kg 75 mg bid or 30 mg cap + 45 mg cap bid as alternative to susp. Childn ≥1 yr weighing >40 kg 75 mg (12.5 mL) bid for 5 days, >23-40 kg 60 mg (10 mL) bid for 5 days, >15-23 kg 45 mg (7.5 mL) bid for 5 days, ≤15 kg 30 mg (5 mL) bid for 5 days. Childn 0-12 mth 3 mg/kg bid for 5 days. Childn <1 yr weighing 10 kg 30 mg (5 mL) bid for 5 days, 9 kg 27 mg (4.5 mL) bid for 5 days, 8 kg 24 mg (4 mL) bid for 5 days, 7 kg 21 mg (3.5 mL) bid for 5 days, 6 kg 18 mg (3 mL) bid for 5 days, 5 kg 15 mg (2.5 mL) bid for 5 days, 4 kg 12 mg (2 mL) bid for 5 days, 3 kg 9 mg (1.5 mL) bid for 5 days. Renal impairment w/ CrCl >30-60 mL/min 30 mg bid for 5 days, 10-30 mL/min 30 mg once daily for 5 days. Patients undergoing routine hemodialysis Initially 30 mg prior to start of dialysis if symptoms develop during 48 hr between dialysis sessions. Administer 30 mg after every hemodialysis session. Peritoneal dialysis 30 mg prior to start of dialysis followed by further 30 mg doses every 5 days. Immunocompromised patient Duration: 10 days. Prophylaxis of flu Adult & adolescent 75 mg once daily for 10 days. Initiate w/in 2 days of exposure. Childn ≥1 yr weighing >40 kg 75 mg cap once daily or 30 mg cap + 45 mg cap once daily for 10 days. Alternatively, 12.5 mL susp once daily, >23-40 kg 60 mg (10 mL) once daily for 10 days, >15-23 kg 45 mg (7.5 mL) once daily for 10 days, ≤15 kg 30 mg (5 mL) once daily for 10 days. Renal impairment w/ CrCl >30-60 mL/min 30 mg once daily, 10-30 mL/min 30 mg every other day. Patients undergoing routine hemodialysis Initially 30 mg prior to start of dialysis. Administer 30 mg after every alternate hemodialysis session. Peritoneal dialysis Initially 30 mg prior to start of dialysis followed by further 30 mg doses every 7 days. Immunocompromised patient ≥1 yr Seasonal prophylaxis for 12 wk.
Administration
May be taken with or without food: May be taken w/ meals to enhance tolerability. Cap: May open cap & mix w/ sweetened food products. Take immediately. Powd for oral susp: Dissolve contents in water.
Contraindications
Special Precautions
Convulsion & delirium-like neuropsychiatric events predominantly in childn & adolescents. Monitor closely for signs of abnormal behaviour especially in childn & adolescents. Hereditary fructose intolerance. ESRD (CrCl <10 mL/min) not undergoing dialysis. Severe hepatic impairment. Pregnancy & lactation. Childn <1 yr.
Adverse Reactions
Nausea; headache. Vomiting; pain.
Drug Interactions
Increased exposure to active metabolite w/ probenecid.
