Diabetes burden and associated complications are on the rise in Singapore and contribute to the increasing healthcare expenditure. Insulin biosimilars, with comparable efficacy and safety to the originator biologics, can help reduce both financial and disease burden. INSTRIDE-1/2 studies have proven biosimilar insulin glargine to be non-inferior to reference insulin glargine with no clinically meaningful differences in terms of efficacy and safety in type 1 and type 2 diabetes patients. Whilst INSTRIDE-3 has proven the switch from reference glargine to a cost-effective biosimilar insulin glargine to be a safe and feasible approach. There is a continued need to educate clinicians to help motivate patients and improve their awareness on biosimilars.

Hyponatraemia has been associated with poor outcomes in patients with heart failure (HF).¹ Tolvaptan is an oral, selective V2 receptor antagonist, indicated for the treatment of clinically significant hypervolaemic or euvolaemic hyponatraemia (associated with HF or syndrome of inappropriate antidiuretic hormone secretion [SIADH]) with either a serum Na⁺ concentration <125 mmol/L or less marked hyponatraemia that is symptomatic and resistant to fluid restriction.² Trials have demonstrated the efficacy of tolvaptan for augmenting diuresis when administered with furosemide for management of congestive HF.^2,3 The following case studies illustrate the use of tolvaptan in this setting.

Nosocomial pneumonia serves as a paradigm for all severe infections, and appropriate antibiotic therapy is the key determinant of patient outcomes. Most data for the use of antibiotics are derived from studies in non-ICU patients and ignore critical care issues such as fluid resuscitation, mechanical ventilation, and changing renal function. Against the backdrop of increasing antimicrobial resistance and the COVID-19 pandemic, how do we optimize antibiotic administration in the ICU? Professor Andrew Shorr from Washington Hospital Center, Georgetown University, Washington DC, US discussed this burning issue at a Pfizer-sponsored satellite symposium, held during the Asia Pacific Intensive Care Symposium (APICS) 2021.
