In combination w/ fludarabine as conditioning treatment prior to allogeneic haematopoietic stem cell transplantation (alloHSCT) in adults w/ AML or myelodysplastic syndrome (MDS) at increased risk of toxicity with standard conditioning therapies, & in paed patients >1 mth w/ malignant & non-malignant diseases.

IV Adult w/ AML & MDS In combination w/ fludarabine 10 g/m² daily as 2-hr infusion for 3 consecutive days (days -4, -3, -2) before stem cell infusion (day 0) for total dose of 30 g/m²; fludarabine 30 mg/m² daily as 0.5-hr infusion for 5 consecutive days (days -6, -5, -4, -3, -2) before stem cell infusion (day 0) for total dose of 150 mg/m². Administer treosulfan before fludarabine on days -4, -3, -2 (FT₁₀ regimen). Ped >1 mth In combination w/ fludarabine, thiotepa (intensified regimen; FT_10-14TT regimen) or w/o thiotepa (FT_10-14 regimen) 10-14 g/m² daily as 2-hr infusion for 3 consecutive days (days -6, -5, -4) before stem cell infusion (day 0) for total dose of 30-42 g/m²; fludarabine 30 mg/m² daily as 0.5-hr infusion for 5 consecutive days (days -7, -6, -5, -4, -3) before stem cell infusion (day 0) for total dose of 150 mg/m²; thiotepa (intensified regimen) 5 mg/kg bd given as 2 infusions over 2-4 hr on day -2 before stem cell infusion (day 0). Adminsiter treosulfan before fludarabine.

Hypersensitivity. Active non-controlled infectious disease. Severe concomitant cardiac, lung, liver, & renal impairment. Fanconi anaemia & other DNA breakage repair disorders. Administration of live vaccines. Pregnancy.
