Indications/Uses
Prophylaxis & treatment of venous thrombosis & pulmonary embolism. MI & arterial embolism. Prevention of clotting in arterial & heart surgery & cerebral thrombosis. Anticoagulant in blood transfusions, extra-corporal circulation, dialysis & for lab purposes.
Dosage/Direction for Use
Haemodialysis 7,500-12,500 IU/dialysis. IV 5,000-10,000 IU 4 hrly either by bolus inj or continuous infusion in NaCl or dextrose inj. SC 10,000 IU 8 hrly after initial IV bolus of 5,000 IU. Low-dose heparin prophylaxis SC 5,000 IU 2-6 hr pre-op & 8-12 hrly post-op for 10-14 days or until patient is mobile. MI SC 5,000 IU 12 hrly beginning during the 12 hr following the 1st sign of MI. Open heart surgery <2 hr 120 IU/kg/hr for 10-14 days in peri-op prophylaxis to 6 wk in the treatment of established thrombosis. For operations of longer duration, 1½ times the dose should be given. For each 450 mL of blood used, 2,000 IU are needed.
Contraindications
Hypersensitivity. When suitable blood coagulation tests (eg, whole-blood clotting time, partial thromboplastin time) cannot be performed at the required intervals. Any uncontrolled active bleeding state. Current or previous immune-mediated heparin-induced thrombocytopenia (type II). Active major haemorrhage & risk factors for major haemorrhage. Septic endocarditis. Locoregional anesth in patients receiving heparin for treatment rather than prophylaxis. Epidural catheter insertion in patients receiving treatment doses. Contains preservative benzyl alcohol 10 mg/mL; not to be given to premature babies or neonates due to risk of gasping syndrome.
