Indications/Uses
Chronic heart failure where systolic dysfunction is the main problem. Patients w/ ventricular dilatation. Heart failure accompanied by atrial fibrillation. Supraventricular arrhythmias particularly chronic atrial flutter & fibrillation.
Dosage/Direction for Use
Adult & childn >10 yr Rapid oral loading: 750-1,500 mcg as single dose. When there is less urgency or greater risk of toxicity (eg, elderly), loading dose should be given in divided doses 6 hr apart w/ approx ½ the total dose given as 1st dose. Slow oral loading: 250-750 mcg daily for 1 wk followed by appropriate maintenance dose. Maintenance dose: Individualized dosage. Childn ≤10 yr, infant & neonates Individualized dosage. Administer loading dose in divided doses w/ approx ½ the total dose given as the 1st dose & further fractions of total dose given at 4-8 hr intervals, assess clinical response before giving each additional dose. Childn 5-10 yr Loading dose: 25 mcg/kg/24 hr, 2-5 yr Loading dose: 35 mcg/kg/24 hr, term neonate to 2 yr Loading dose: 45 mcg/kg/24 hr. Pre-term neonate weighing 1.5-2.5 kg Loading dose: 30 mcg/kg/24 hr, <1.5 kg Loading dose: 25 mcg/kg/24 hr.
Administration
May be taken with or without food: May split using score on tab but do not divide into equal doses.
Contraindications
Hypersensitivity to digoxin or other digitalis glycosides. Intermittent complete heart block or 2nd-degree AV block especially if w/ history of Stokes-Adams syndrome. Arrhythmias originating through intoxication w/ cardiac glycosides. Supraventricular arrhythmias associated w/ AV accessory pathway (Wolff-Parkinson-White syndrome). Ventricular tachycardia or fibrillation. Hypertrophic obstructive cardiomyopathy.
