Indications/Uses
Management of renal tubular acidosis (RTA) w/ Ca stones; hypocitraturic Ca oxalate nephrolithiasis of any etiology; uric acid lithiasis w/ or w/o Ca stones. Prevention of stone relapse after extracorporeal shock wave lithotripsy (ESWL) or lithotomy.
Dosage/Direction for Use
Adult Severe hypocitraturia (urinary citrate <150 mg/day) 60 mEq/day in 2 or 3 divided oral doses (30 mEq bid or 20 mEq tid). Mild to moderate hypocitraturia (urinary citrate >150 mg/day) 30 mEq/day in 2 or 3 divided oral doses (15 mEq bid or 10 mEq tid).
Administration
Should be taken with food: Take 30 min after meals. Swallow whole, do not crush/chew/suck.
Contraindications
Hypersensitivity. Patients w/ hyperkalemia or conditions predisposing to hyperkalemia eg, chronic renal failure, uncontrolled DM, familial hyperkalemic periodic paralysis, acute dehydration, strenuous physical exercise in unconditioned individuals, adrenal insufficiency, extensive tissue breakdown, severe myocardial damage, administration of K-sparing diuretic (eg, triamterene, spironolactone or amiloride). Slow gastric emptying, esophageal compression, intestinal obstruction or stricture; anticholinergic therapy; active PUD; active UTI; renal insufficiency; resp or metabolic acidosis.
Special Precautions
Avoid in patients w/ chronic renal failure or impaired K excretion eg, severe myocardial damage or heart failure. Discontinue if severe vomiting, abdominal pain or GI bleeding occurs & investigate possibility of bowel perforation or obstruction. Monitor serum K regularly. Discontinue if hyperkalemia occurs. Pregnancy & lactation. Childn.
Adverse Reactions
Minor GI complaints eg, abdominal discomfort, vomiting, diarrhea, loose bowel movement & nausea.
Drug Interactions
Severe hyperkalemia w/ K-sparing diuretics (eg, triamterene, spironolactone or amiloride). Increased GI irritation w/ drugs that slow GI transit time (eg, anticholinergics).
