Gentamicin sulfate
Indications/Uses
Bacteremia, septicemia, UTI, severe chest infections & other systemic infections due to susceptible bacteria.
Dosage/Direction for Use
Adult 3-4 mg/kg daily in divided doses. Typical doses: ≥60 kg 80 mg 8 hrly, <60 kg 60 mg 8 hrly. Renal impairment CrCl >70 mL/min 80 mg 8 hrly, 30-70 mL/min 80 mg 12 hrly, 10-30 mL/min 80 mg daily, 5-10 mL/min 80 mg every 48 hr, <5 mL/min 80 mg after dialysis. UTI 160 mg once daily. Increase dose to 6 hrly or for a total dose of 5 mg/kg in 24 hr in cases of life-threatening infections. Childn up to 12 yr 2 mg 8 hrly. Infant up to 2 wk 3 mg 12 hrly.
Contraindications
Hypersensitivity. Myasthenia gravis.
Special Precautions
Impaired renal function. Assess CrCl rates or blood urea. Check serum levels to confirm that peak (1 hr) levels do not exceed 10 mcg/mL & trough levels (before next inj) do not exceed 2 mcg/mL. Pregnancy & lactation.
Adverse Reactions
Drug Interactions
Potential increase of nephrotoxicity w/ cephalosporins. May potentiate ototoxicity w/ furosemide, piretanide & ethacrynic acid. Reduced activity w/ ticarcillin. Neuromuscular blockade & resp paralysis w/ curare-type muscle relaxants during anesth. Antagonism w/ bacteriostatic antibiotics.
