Indications/Uses
Treatment & prevention of Fe & folate deficiency anemia.
Dosage/Direction for Use
Special Precautions
Fe: Not to be given to patients receiving repeated blood transfusions or to patients w/ anemias not produced by Fe deficiency unless Fe deficiency is also present. Do not administer oral Fe therapy concomitantly w/ parenteral Fe. Patients w/ Fe-storage or Fe-absorption diseases eg, hemochromatosis, hemoglobinopathies, or existing GI diseases eg, inflammatory bowel disease, intestinal strictures & diverticulae. Folic acid: Never to be given alone or in conjunction w/ inadequate amounts of vit B12 for the treatment of undiagnosed megaloblastic anemia, since folic acid may produce hematopoietic response in patients w/ megaloblastic anemia due to vit B12 deficiency w/o preventing aggravation of neurological symptoms.
Adverse Reactions
Fe: GI irritation & abdominal pain w/ nausea & vomiting when administered orally. Other GI effects may include either diarrhea or constipation. Folic acid: GI disturbances & hypersensitivity reactions (rarely).
Drug Interactions
Fe: Administration w/ food may further impair Fe salts absorption. Compd containing Ca & Mg, including antacids & mineral supplements, & bicarbonates, carbonates, oxalates, or phosphates, may also impair absorption of Fe by formation of insoluble complexes. Absorption of both Fe salts & tetracyclines is diminished when taken concomitantly PO. A suitable interval is advised if Fe supplement is required in patients receiving trientine. Zn salts may decrease absorption of Fe. Ascorbic acid & citric acid may increase absorption of Fe. Delayed response to Fe in patients receiving systemic chloramphenicol. Folic acid: Folate deficiency states may be produced by antiepileptics, OCs, antituberculous drugs, alcohol, & folic acid antagonists eg, aminopterin, MTX, pyrimethamine, trimethoprim & sulfonamides. Decreased serum-phenytoin conc & possibility that same effect could also occur w/ barbiturate antiepileptics.
