Simvastatin
Indications/Uses
Reduction of LDL-C, Apo B, & triglycerides, & increase HDL-C in the treatment of hyperlipidaemias including hypercholesterolaemias, combined (mixed) hyperlipidaemia (type IIa or IIb hyperlipoproteinaemias), hypertriglyceridemia (type IV) & primary dysbetalipoproteinaemia (type III); adjunct therapy in patients w/ HoFH who have some LDL-receptor function; CV risk reduction in patients w/ ASCVD or DM.
Dosage/Direction for Use
Primary hypercholesterolaemia, combined hyperlipidaemia 10-20 mg daily at night, adjusted at intervals of at least 4 wk. Usual range: 10-80 mg once daily at night. HoFH 40 mg daily at night or 80 mg daily in 3 divided doses (largest dose at night). Prevention of CV events Initially 20-40 mg daily at night, adjusted at intervals of at least 4 wk; max: 80 mg once daily at night. Concomitant use w/ ciclosporin, fibrate or lipid-lowering dose of nicotinic acid Max: 10 mg daily, w/ amiodarone or verapamil Max: 20 mg daily, w/ diltiazem Max: 40 mg daily.
Administration
May be taken with or without food.
Special Precautions
Not to be given to patients w/ acute liver disease or unexplained persistent raised serum-aminotransferase conc. Discontinue if marked increase in serum-aminotransferase or creatine phosphokinase conc occur. Avoid during pregnancy since there is a possibility to interfere w/ fetal sterol synthesis. 10 mg & 20 mg: Contains tartrazine which may cause allergic reaction (including bronchial asthma) in certain susceptible person.
Adverse Reactions
GI disturbances. Headache, skin rashes, dizziness, blurred vision & dysgeusia. Reversible increases in serum-aminotransferase conc. Myopathy, characterized by myalgia & muscle weakness & associated w/ increase creatine phosphokinase conc, especially in patients taking immunosuppressive drugs, fibric acid derivatives or nicotinic acid. Rarely, rhabdomyolysis w/ acute renal failure.
Drug Interactions
Increased risk of myopathy w/ ciclosporin, itraconazole, ketoconazole, erythromycin, clarithromycin, HIV-PIs, & nefazodone; fibric acid derivatives or nicotinic acid.
