Bupivacaine HCl

Indications/Uses

Spinal anesth for urological & lower limb surgery lasting 2-3 hr, abdominal surgery lasting 45-60 min.

Dosage/Direction for Use

Adult Urological surgery 7.5-15 mg. Lower limbs (including hip) & abdominal (including caesarean section) surgery 10-20 mg.

Overdosage

View Regivell overdosage for action to be taken in the event of an overdose.

Contraindications

Hypersensitivity to bupivacaine HCl or amide-type local anaesth. Active CNS disease eg, meningitis, poliomyelitis, intracranial hemorrhage, subacute combined cord degeneration due to pernicious anemia, cerebral & spinal tumours. Spinal stenosis & active disease eg, spondylitis, TB, tumour or recent trauma eg, vertebral column fracture; septicaemia; pyogenic skin infection at or adjacent to lumbar puncture site; cardiogenic or hypovolaemic shock; coagulation disorders or ongoing anticoagulant therapy.

Special Precautions

Not to be inj intravascularly. Discontinue use if signs of acute systemic toxicity or total spinal block appear. Patients w/ hypovolemia; in poor general condition due to ageing or other compromising factors eg, partial/complete heart conduction block; pleural effusions. Neurological injury. May cause hypotension & bradycardia; intercostal paralysis. Concomitant use w/ class III anti-arrhythmic drugs. May affect ability to drive & use machines. Advanced liver or renal dysfunction. Not to be given in early pregnancy & reduce dose in late stages of pregnancy. Elderly.

Hypotension, bradycardia; nausea. Postdural puncture headache; vomiting; urinary retention & incontinence.

Drug Interactions

Additive systemic toxic effects w/ other local anaesth or structurally-related amide-type local anaesth eg, lidocaine, mexiletine. Caution w/ class III anti-arrhythmic drugs eg, amiodarone.
