Indications/Uses
Management of neuropathic pain associated w/ diabetic peripheral neuropathy (DPN); postherpetic neuralgia (PHN).
Dosage/Direction for Use
Neuropathic pain associated w/ DPN Initially 165 mg once daily, may be increased to 330 mg once daily w/in 1 wk. Max: 330 mg once daily. PHN Initially 165 mg once daily, may be increased to 330 mg once daily w/in 1 wk. May be further increased to 660 mg once daily if no sufficient pain relief following 2-4 wk treatment w/ 330 mg once daily. Max: 660 mg once daily. Renal impairment CrCl 30-60 mL/min 82.5-330 mg once daily.
Administration
Should be taken with food: Swallow whole, do not split/crush/chew.
Contraindications
Special Precautions
Discontinue use if hypersensitivity; life-threatening angioedema; diagnosed or suspected myopathy or markedly elevated creatine kinase levels occur. Monitor for emergence or worsening of depression, suicidal thoughts or behavior &/or unusual changes in mood or behavior; symptoms of resp depression & sedation. Dizziness & somnolence; peripheral edema; CHF patients w/ NYHA class III or IV; wt gain; tumorigenic potential; visual disturbances; decreased platelet count; prolonged PR interval; encephalopathy. Monitor patients w/ history of substance abuse; symptoms of misuse, abuse or dependence eg, development of tolerance, dose escalation, drug-seeking behavior. Avoid abrupt or rapid discontinuation; taper-off gradually over min of 1 wk. Concomitant use w/ thiazolidinedione antidiabetics; anti-spasticity agents. Not to be taken by patients w/ galactose intolerance, Lapp-lactase deficiency or glucose-galactose malabsorption. May affect ability to drive & use machines. Not recommended in patients w/ CrCl <30 mL/min or undergoing haemodialysis. Renal impairment. Women of childbearing potential must use effective contraception. Not to be used during pregnancy. Not recommended during lactation. Not recommended in childn <12 yr & adolescents. Elderly >65 yr.
Adverse Reactions
Dizziness, somnolence, headache. Nasopharyngitis; increased appetite; euphoric mood, confusion, irritability, depression, disorientation, insomnia, decreased libido; ataxia, abnormal coordination, tremor, dysarthria, amnesia, memory impairment, attention disturbance, paraesthesia, hypoesthesia, sedation, balance disorder, lethargy; blurred vision, diplopia; vertigo; vomiting, constipation, flatulence, abdominal distension, dry mouth, nausea, diarrhoea; muscle cramp, arthralgia, back & limb pain, cervical spasm; peripheral oedema, oedema, abnormal gait, fall, feeling drunk & abnormal, fatigue; increased wt.
Drug Interactions
Fatal resp depression w/ other CNS depressants eg, opioids.
