Indications/Uses
Major depressive episodes (ie, typical).
Dosage/Direction for Use
1 tab (12.5 mg) tds. Elderly >70 yr & patient w/ renal insufficiency Max: 2 tab/day.
Administration
Should be taken with food: Take at the beginning of main meals.
Contraindications
Hypersensitivity. Concomitant use w/ MAOIs; a wash-out period of 2 wk is necessary between treatment w/ MAOIs & treatment w/ tianeptine; a wash-out period of 24 hr is required when replacing tianeptine w/ MAOI. Childn & adolescent <15 yr.
Special Precautions
Closely monitor patients for clinical worsening or appearance of suicidal thoughts/behaviour & any abnormal change of behaviour. If general anaesth is necessary, discontinue tianeptine 24 or 48 hr prior to surgery. In case of emergency surgery, pre-op monitoring should be performed. Should not be taken w/ alcoholic beverages or medicines containing alcohol. In case of treatment interruption, gradually reduce dose over 7-14 days. Patients w/ history of drug or alcohol dependence. Patients w/ rare hereditary problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency. May impair ability to drive & operate machinery. Pregnancy. Breast-feeding is not recommended during treatment. Should not be used in adolescents 15-18 yr.
Adverse Reactions
Anorexia; nightmare; insomnia, somnolence, dizziness, headache, lipothymia, tremor; tachycardia, extrasystoles, chest pain; hot flush; dyspnoea; gastralgia, abdominal pain, dry mouth, nausea, vomiting, constipation, flatulence; myalgia, lumbar pain; asthenia, lump feeling in throat.
Drug Interactions
Risk of CV collapse or paroxysmal HTN, hyperthermia, convulsions, death w/ irreversible MAOIs (eg, iproniazide).
