Phenoxymethylpenicillin
Indications/Uses
Treatment of mild to moderately severe infections associated w/ microorganisms whose susceptibility to penicillin is w/in the range of serum levels attained w/ the dosage form. Streptococcal infections (w/o bacteraemia): Mild to moderate infections of the upper resp tract, scarlet fever & mild erysipelas. Pneumococcal infections: Mild to moderately severe infections of the resp tract. Staphylococcal infections: Mild infections of the skin & soft tissues. Fusospirochaetosis (Vincent's gingivitis & pharyngitis): Mild to moderately severe infections of the oropharynx. Prophylaxis against recurrence of rheumatic fever & chorea.
Dosage/Direction for Use
Adult (including the elderly) & childn >12 yr 250-500 mg 6 hrly. Long term prophylaxis of rheumatic fever 250 mg bd. Childn 6-12 yr 250 mg 6 hrly, 1-5 yr 125 mg 6 hrly, infants (up to 1 yr) 62.5 mg 6 hrly.
Administration
Should be taken on an empty stomach: Take 30 min before or at least 3 hr after meals.
Contraindications
Hypersensitivity to penicillins. Rare hereditary problems of fructose intolerance, glucose-galactose malabsorption or sucrase-isomaltase insufficiency.
Special Precautions
History of significant allergies &/or asthma. History of sensitivity to penicillins, cephalosporins & other allergens. Patients w/ severe illness, or w/ nausea, vomiting, gastric dilation, achalasia or intestinal hypermotility. Presence of markedly impaired renal function. Prolonged use may promote over growth of non-susceptible organisms, including fungi. Pregnancy & lactation.
Adverse Reactions
Nausea, vomiting, abdominal pain, diarrhoea; allergic reactions; pseudomembranous colitis; urticarial, erythematous or mobilliform rash & pruritus.
Drug Interactions
Reduced absorption w/ neomycin, or guar gum. Antagonised bactericidal activity w/ bacteriostatic antibiotics eg, chloramphenicol, erythromycin & tetracyclines. Reduced excretion of methotrexate. Reduced efficacy of combined OCs. Reduced excretion w/ probenecid, or sulfinpyrazone. May interfere w/ anticoagulants. May inactivate oral typhoid vaccine.
