Theophylline

Indications/Uses

Relief & prophylaxis of reversible bronchospasm associated w/ bronchial asthma, bronchitis, emphysema, & related conditions.

Dosage/Direction for Use

SR tab Adult 200-300 mg 12 hrly. Childn >2 yr 10 mg/kg body wt 12 hrly. Syr Adult & childn >12 yr 25 mL 6 hrly. Childn 6-12 yr 20-25 mL 6 hrly, 4-6 yr 15-20 mL 6 hrly, 2-4 yr 10-15 mL 6 hrly.

Administration

Should be taken with food: SR tab: Swallow whole, do not chew/crush.

Special Precautions

Monitor plasma levels. Acute asthma symptoms, xanthine medications/beverages, reduced thyroid functions, CHF, acute pulmonary oedema, COPD, severe hypoxia, pneumonia, acute febrile episodes, acute viral infection, impaired liver function. Cardiac arrhythmias, CAD, unstable angina, cardiomyopathy, severe HTN, peptic ulcer, GERD. Elderly & debilitated patients. Pregnancy, lactation.

Adverse Reactions

Gastric irritation, nausea, vomiting, anorexia, epigastric pain, reactivation of peptic ulcer, gastro-oesophageal reflux, haematemesis, tachycardia, palpitation, headache, CNS stimulation, reflex hyperexcitability, insomnia, tremor.

Drug Interactions

Cimetidine, allopurinol (high dose), propranolol, macrolides, quinolones, alcohol, OC, mexiletine, tacrine, thiabendazole, disulfiram, interferon α, verapamil, phenobarb, phenytoin, carbamazepine, rifampicin, halothane, ketamine, lithium, sympathomimetics & St. John's Wort. Caution when used w/ ranitidine, diltiazem, nifedipine, INH, frusemide, influenza vaccine, corticosteroids.
