Progesterone
Indications/Uses
Premenstrual syndrome, including PMT & depression; puerperal depression. Luteal phase support as part of an Assisted Reproductive Technology (ART) treatment for women.
Dosage/Direction for Use
Premenstrual syndrome & puerperal depression 200-400 mg once daily-bd, by vag or rectal insertion. Premenstrual syndrome Commence treatment on day 14 of menstrual cycle & continue treatment until onset of menstruation. If symptoms are present at ovulation commence treatment on day 12. Luteal phase support as part of an ART treatment 400 mg bd by vag insertion, starting at oocyte retrieval. Continue for 38 days for confirmed pregnancy.
Contraindications
Hypersensitivity. Undiagnosed vag bleeding; known/suspected progesterone sensitive malignant tumours; porphyria. History or active arterial thromboembolism or VTE or severe thrombophlebitis. Severe hepatic dysfunction/disease. Known missed abortion/ectopic pregnancy.
Special Precautions
Discontinue in the event of a missed miscarriage; in case of suspected MI, cerebrovascular disorders, arterial thromboembolism or VTE, thrombophlebitis or retinal thrombosis; if symptoms of depression worsens. Not indicated in threatened miscarriage. Carefully observe patients w/ epilepsy, migraine, asthma, cardiac or renal dysfunction, or diabetes. Patients w/ hormone-sensitive conditions (eg, 2nd half of menstrual cycle & pregnancy). Increased anxiety, moodiness & sensibility to seizures in abrupt progesterone discontinuation. Use rectally if barrier methods of contraception are used or if patients suffer from vag infection (especially moniliasis) or recurrent cystitis or have recently given birth. Use vaginally if patients suffer from colitis or fecal incontinence. Hepatic dysfunction. Not to be used during pregnancy & lactation.
Adverse Reactions
Somnolence; abdominal pain & discomfort, constipation; breast pain; hot flush; fatigue.
Drug Interactions
Decreased bioavailability w/ drugs known to induce hepatic CYP450 3A4 system (eg, rifampicin, carbamazepine or phenytoin). Not recommended w/ other vag products.
